ABSTRACT

Resumo Coarctação da aorta abdominal é uma causa rara de claudicação de membros inferiores e hipertensão refratária. O tratamento é complexo e exige conhecimento de diversas técnicas de reconstrução vascular. Apresentamos um caso de coarctação ao nível das artérias renais, seu tratamento e revisão da literatura. Paciente feminina, 65 anos, com hipertensão refratária desde os 35 anos, utilizando cinco medicações anti-hipertensivas em dose máxima. Pressão arterial média de 260/180mmHg e claudicação incapacitante (menos de 20 metros) bilateral. Angiotomografia computadorizada demonstrou coarctação de aorta justarrenal de 4 mm de maior diâmetro, calcificação circunferencial no local da estenose e tortuosidade da aorta infrarrenal. Foi submetida a tratamento híbrido, com ponte ilíaco-birrenal e implante de stent Advanta V12 no local da estenose. A paciente evoluiu satisfatoriamente e, 60 dias depois da cirurgia, apresentava-se com uma pressão arterial de 140/80mmHg, em uso de apenas duas medicações anti-hipertensivas e sem claudicação.


Abstract Coarctation of the abdominal aorta is a rare etiology of intermittent claudication and refractory hypertension. Treatment is complex and requires knowledge of several vascular reconstruction techniques. We report a case of aortic coarctation at the level of the renal arteries, describing its treatment and presenting a literature review. Female patient, 65 years old, with refractory hypertension since the age of 35, using five antihypertensive medications at maximum doses. Blood pressure was 260/180mmHg and she had disabling claudication (less than 20 meters). Computed tomography angiography showed a 4mm coarctation in the juxtarenal aorta, with circumferential calcification at the stenosis site, and tortuous infrarenal aorta. Hybrid repair was performed with an iliac-birenal bypass and implantation of an Advanta V12 stent at the stenosis site. The patient's postoperative course was satisfactory, she was free from claudication, and her blood pressure 60 days after surgery was 140/80mmHg, taking two antihypertensive medications.

ABSTRACT

Adventitial cystic disease of the popliteal artery is a rare condition of uncertain etiology, which presents as intermittent claudication of the lower extremity in middle-age patients. We report a 44-year-old man presenting with intermittent claudication of his left leg. MR angiography showed cystic parietal lesions that caused compression with partial occlusion of the left popliteal artery. Surgical resection of the affected segment was performed, with venous graft interposition. The histopathological analysis of the surgical specimen was consistent with cystic adventitial disease.

ABSTRACT

A claudicação intermitente está frequentemente associada à doença aterosclerótica, mas diagnósticos diferenciais devem ser pesquisados em pacientes sem fatores de risco tradicionais. A doença cística adventicial, de etiologia incerta, acomete em maior proporção a artéria poplítea e, eventualmente, apresenta-se como claudicação intermitente. Apresentamos um caso da doença e seu manejo cirúrgico, e discutimos a etiopatogenia, os aspectos diagnósticos e terapêuticos da enfermidade.


Intermittent claudication is frequently associated with atherosclerotic disease, but differential diagnosis must be sought in patients with no traditional risk factors. Cystic adventitial disease, of unknown etiology, most frequently affects the popliteal artery, and occasionally presents as intermittent claudication. We report a case of this disease and the surgical treatment, and discuss some aspects related to etiopathogenesis, diagnosis and treatment of this condition.

OBJETIVO: Verificar se há relação entre a medida da capacidade funcional avaliada subjetiva e objetivamente em relação ao estado atual e após tratamento clínico em pacientes com claudicação intermitente. MÉTODOS: Foram recrutados 500 pacientes com claudicação intermitente. Todos realizaram exame clínico e foram submetidos a uma avaliação funcional por meio do método subjetivo (consulta clínica) e objetivo (teste de esteira). Adicionalmente, 50 pacientes foram selecionados para se avaliar o efeito do tratamento clínico, pelos métodos subjetivo e objetivo, em relação à capacidade funcional. RESULTADOS: Dos 500 pacientes selecionados, somente 60 (12,0%) apresentaram valores referidos e observados similares. Os demais pacientes, ou seja, 440 (88,0%) apresentaram valores relatados discordantes em relação àqueles obtidos no teste de esforço. Com relação ao efeito do tratamento clínico em relação à capacidade funcional, os resultados foram similares entre ambos os métodos (χ²=1,7; p=0,427). CONCLUSÃO: Apesar de o método subjetivo superestimar os valores de capacidade funcional, quando comparados ao método objetivo, não foram observadas diferenças significantes entre ambos os métodos, quando se analisou o efeito do tratamento clínico. Assim, o método subjetivo fornece informações similares em comparação à medida objetiva no acompanhamento do tratamento clínico de pacientes com claudicação intermitente.


OBJECTIVE: To analyze if there is any relation between functional capacity assessed by subjective and objective methods regarding the current state and after clinical treatment in patients with intermittent claudication. METHODS: A total of 500 patients with intermittent claudication were enrolled. All patients underwent clinical examination and a functional evaluation by subjective (clinical visit) and objective method (treadmill test). Additionally, 50 patients were selected to evaluate the effect of clinical treatment by subjective and objective methods in relation to functional capacity. RESULTS: Out of 500 patients, only 60 (12.0%) had similar results in both methods. The remaining, that is 440 patients (88.0%) had subject values in disagreement with stress test findings. Regarding the clinical effect of the treatment on the functional outcomes, results were similar in both methods (χ²=1.7; p=0.427). CONCLUSION: Although the subjective method overestimates the functional capacity when compared to the objective method, no significant differences were observed between both methods when analyzing the effect of clinical treatment. Thus, the subjective method provides similar information as compared with objective method, in monitoring the clinical treatment of patients with intermittent claudication.

ABSTRACT

La revascularización endovascular de la aorta infrarrenal en pacientes sintomáticos con lesiones oclusivas ateroescleróticas crónicas surge como modalidad terapéutica en casos seleccionados, idealmente en lesiones tipo A y B. Comunicar los resultados inmediatos y el seguimiento a largo plazo, de pacientes con clínica de claudicación intermitente de miembros inferiores, diagnosticados con estenosis significativa de la aorta infrarrenal entre junio de 2002 y enero de 2008. Tres pacientes fueron sometidos a angioplastía percutánea de la aorta infrarrenal. con implante de stent o stent-graft, indicándose al egreso aspirina, clopidogrel y cilostazol, así como rehabilitación. Se realizó seguimiento clínico y por angiotomografía al año. El promedio de estenosis fue de 86,3%, 0% posangioplastia. Durante el seguimiento se evidenció mejoría clínica y ausencia de reestenosis. La angioplastia con balón e implantación de stent ha demostrado ser una técnica segura y efectiva como lo describe la literatura actualmente. Siendo una modalidad de menor costo para pacientes con esta entidad clínica.


Endovascular revascularization of chronic atherosclerotic type A and B lesions of the infrarenal aorta is a therapeutic option in selecte symptomatic patients. Communicate immediate and long-term follow-up autcome of patients with lower limb intermittent claudication with significant infrarenal aortic stenosis diagnosis between June 2002 and January 2008. Three patients underwent percutaneous transluminal angioplasty of the infrarenal aorta with either stent or stent-graft deployment, treated ofter discharge with aspirin, clopidogret and rehabilitation. One year clinical and tomographic follow up was made. Stenosis average was 86.3%, 0% after angioplasty. Clinical improvement and no restenosis were evident during follow-up. Percutaneous transluminal angioplasty with stent deployment seems to be a safe an effective approach to treat these lesions as reported in the literature, at a lower cost on an outpatient basis.

ABSTRACT

La insuficiencia arterial de extremidades inferiores es la forma más frecuente de enfermedad arterial periférica. Clínicamente se presenta como un cuadro asintomático, como claudicación intermitente o finalmente como isquemia crítica de la extremidad. Esta última instancia, caracterizada por dolor de reposo, gangrena o úlcera isquémica de la extremidad, progresará hacia la amputación de no mediar una intervención. El tratamiento médico es básico en todos estos pacientes. Se deben corregir los factores de riesgo, poniendo énfasis en la suspensión del cigarrillo y buen control de la hipertensión arterial, dislipidemia y diabetes. El ejercicio de marcha supervisado es primordial. Hoy en día las opciones de revascularización son la cirugía abierta tradicional y la terapia endovascular. La decisión sobre cuál es ofrecida a los pacientes se basa en variables anatómicas de la lesión y del estado general del paciente. El ideal es tratar a todos los pacientes por vía endovascular dado la baja morbi mortalidad y rápida recuperación postoperatoria.


Peripheral vascular disease commonly affects the arteries supplying the legs. Most of these patients are asymptomatic, some complain of intermittent claudication and luckily few present with critical limb ischemia (rest pain, ulceration and gangrene). In the latter amputation is the only solution unless the limb is revascularised. Medical treatment is essential, based on control of risk factors as hypertension, diabetes and lipid disorders. All patients should be advice to stop smoking and take a supervised exercise program. Nowadays the options for revascularization are endovascular procedures and surgery. Which one is suitable for every patient depends on anatomical and patient characteristics; however the best one seems to be the endovascular procedure due to low morbidity and mortality plus fast recovery

ABSTRACT

OBJETIVO: O objetivo deste trabalho é apresentar um método para a avaliação da limitação funcional causada por doença arterial oclusiva de artéria subclávia: o teste de esforço utilizando o dinamômetro isocinético. MÉTODO: Pacientes com trombose unilateral de artéria subclávia foram selecionados, reunindo 23 pacientes no Grupo com doença arterial oclusiva de artéria subclávia. Sete pacientes com idade semelhante, sem doença arterial em membros superiores foram incluídos, formando o grupo controle. Para a realização do teste, utilizou-se o dinamômetro isocinético CYBEX® 6000. O cotovelo foi testado em séries consecutivas de 30 repetições do movimento de extensão e flexão, até que se atingisse o máximo de 270 repetições (9 séries), ou até que se alcançasse o limite do membro testado. RESULTADO: Inicialmente comparou-se todos os membros sem doença arterial dos dois grupos, para analisar se apresentavam desempenho semelhante. Não houve diferença estatística entre os grupos em relação a todos os parâmetros estudados. Comparou-se então, os dois membros de cada paciente do Grupo doença arterial oclusiva de artéria subclávia. Em todos os parâmetros analisados, os membros com doença arterial oclusiva de artéria subclávia apresentaram diferença estatística (p < 0.05) em relação aos membros controle, o que foi objetivamente atribuído à isquemia. (Foram registrados diferentes graus de limitação entre os paciente, o que permite estimar objetivamente o grau de limitação causado pela isquemia causada pela oclusão da subclávia).CONCLUSÃO: Este teste de esforço permite que pacientes com isquemia de membros superiores sejam avaliados e estratificados, conforme o grau de sua limitação funcional, o que facilitará a escolha da melhor terapêutica para cada caso e a obtenção de parâmetros para comparação do resultado de diferentes tratamentos e para o seguimento clínico em longo prazo.

ABSTRACT

A 32 years old female was admitted to hospital due to acute abdominal pain, nausea, vomiting and liquid stools. Physical examination was normal except for pain on her left inferior abdominal quadrant without peritoneal irritation signs. An abdominal CAT-scan suggested thrombosis at celiac trunk, although the echo Doppler showed no alterations except for signs of ischemia in the distal branch of the superior mesenteric artery. An exploratory laparotomy was performed disclosing a necrosis of the distal ileum and cecum, diffuse peritonitis and thrombosis of the ileocecoapendiculocolic artery. No vasculitis lesions were found in the arteries of medium size examined. A history of intermittent claudication for the past 3 years as well as acrocyanosis, asymmetry of pulses and blood pressure in the superior extremities was ascertained after the surgery. A MRI angiogram showed multiple stenoses and irregularities at the celiac trunk, hepatic, superior mesenteric and fibular arteries. No abnormalities at the aortic arch and its main branches were documented. A sepsis due to Candida sp complicated her postoperatory period. After recovery, prednisone 1 mg/kg/day was started and the anticoagulation continued. The abdominal pain, intermittent claudication and superior limb acrocyanosis disappeared. This is an unusual case of type IV Takayasu's arteritis with acute abdominal signs as the first manifestation

ABSTRACT

Lumbosacral corset is a spinal support widely used for patients suffering from low back pain due to various conditions. To evaluate the effectiveness of the corset in symptomatic degenerative lumbar spinal stenosis, twenty one patients (mean age 62.5 +/- 5.2 years) with neurogenic claudication (mean onset 9.0 +/- 9.3 months) were recruited for and completed a self controlled comparative study (with and without a corset). Quantitative and qualitative assessment in terms of walking distance and pain score (0-10 point visual analog scale) in daily activities respectively, compared between wearing and not wearing the corset were measured. The outcome of the study showed statistically significant improvement in walking distance (393.2 +/- 254.0 m and 314.6 +/- 188.8 m) and decrement of pain score in daily activities (4.7 +/- 1.4 and 5.9 +/- 1.0) with and without corset dressing respectively. This result supports the positive effect of the lumbosacral corset in pain relief and functional improvement of the degenerative lumbar spinal stenosis condition.

ABSTRACT

This paper attempts to distil some of the results of vasculopathy studies performed on Jamaican diabetic clinic attendees. Doppler measurements of ankle/brachial pressure index (A/BI) revealed that 23of the diabetics had peripheral occlusive arterial disease (POAD) which was mostly asymptomatic. Plethysmographic blood flow studies revealed a profound reduction in the vasodilatory response to increased flow demand. Prevalence of POAD determined by Doppler testing of A/BI reported by other researchers ranged from 13in a large community study, one-third of whom were diabetic, to 47in patients who had been diabetic for 20 years. Isolated posterior tibial disease has been reported to carry a three-fold risk of all cause mortality and a four-fold risk of coronary heart disease mortality. This underscores the need for regular Doppler A/BI testing in order to improve the recognition, and treatment of POAD, and prevent further cardiovascular morbidity and mortality.

ABSTRACT

Se evaluaron los resultados obtenidos en el manejo quirúrgico de los pacientes con enfermedad aorto-iliaca obstructiva tratados en el CMN 20 de Noviembre ISSSTE. De junio de 1995 a junio de 1998 se operaron 20 pacientes; quince masculinos (75 por ciento) y cinco femeninos (25 por ciento), con una variación de edad de 48-74 años (media 66.3 años). Factores de riesgo: tabaquismo 90 por ciento, diabetes mellitus 65 por ciento, hipertensión arterial 40 por ciento, cardiopatía isquémica 20 por ciento. Indicaciones de cirugía-isquemia crítica 14 pacientes (70 por ciento) y claudicación intermitente incapacitante 6 pacientes (30 por ciento). A todos los pacientes se les realizó derivación aorto-bifemoral. Cinco pacientes (20 por ciento) con enfermedad aorto-iliaca tipo III requirieron derivación secuencial distal, 3 a la arteria poplítea por debajo de la rodilla y dos a la tibial anterior. El tiempo quirúrgico promedio fue de 4.15 horas con pinzamiento aórtico promedio de 42.7 min. En el grupo de pacientes con isquemia crítica la mortalidad operatoria fue de 21 por ciento (3 pacientes) por infarto agudo al miocardio (AIM) en un paciente, coagulopatía en otro, e insuficiencia renal aguda e infarto agudo al miocardio en el tercero. A los pacientes a quienes se realizó derivación distal ninguno falleció y los injertos continúan permeables. En el grupo de pacientes operados por claudicación intermitente incapacitante se presentó mortalidad en un paciente (16 por ciento), ocasionado por infarto agudo al miocardio. Durante el seguimiento de 2-38 meses (22.2 meses), fallecieron 2 pacientes uno por cáncer (Ca) broncopulmonar y otro por neuroinfección. En los 14 pacientes vivos sus prótesis permanecen clínicamente y por Doppler permeables.

Antecedentes: Se destaca la particular importancia de estas lesiones que pueden ocasionar la pérdida del miembro. Objetivo: Se hace un estudio retrospectivo de los casos operados por patología de la arteria poplítea. En el mismo se incluyen la patología obliterante por arteriosclerosis, los aneurismas poplíteos, las embolias, los traumatismos y las enfermedades poco frecuentes. Material: Se efectuaron 435 operaciones por patología obliterante arterioesclerótica. Método: De preferencia se utilizó el autoinjerto venoso y los resultados fueron favorables en el 83,2 por ciento de los casos. Se operaron 52 aneurismas poplíteos, el 50 por ciento asintomáticos y los restantes con isquemia aguda o hemorragia. Resultados: Fueron buenos en el 96,1 por ciento de los casos. se destaca la importancia de operar estos aneurismas aún asintomáticos por el alto porcentaje de complicaciones que pueden presentar, especialmente trombosis. Se operaron 66 embolias de la arteria poplítea. La principal fuente embolígena fue el corazón con fibrilación auricular. En este grupo de pacientes hubo un porcentaje alto de amputaciones, 19,6 y 6 por ciento de mortalidad. En los 19 casos de traumatismo de arteria poplítea operados hubo un 10,5 por ciento de amputaciones. Se resalta la importancia del diagnóstico temprano, especialmente en los traumatismos cerrados. Entre las enfermedades poco frecuentes se presentan 2 aneurismas infecciosos, 1 aneurisma arteriovenoso, 2 casos de enfermedad adventicial quística y 2 de síndrome de atrapamiento.
